The Form.com application provides you with 14 question types to collect critical data. This page describes the meaning and purpose of each question type available.

Use this question type to write introductions or give additional headings to blocks of questions in your form. Your text can contain long tables, images, etc. There is no answer option, since this question type requires no answers. | ||
Use this question type when you want your respondents to choose from several answer options. The option 'Other' allows respondents to add their own answer option in case none of the predefined answer options suits. This question type requires at least 2 answer options. This question type allows the respondent to enter several individual lines of text which will correspond to question parts. You can also require the answer option entry meet certain requirements using the answer format option (pre-formatting can be used to make sure the entry is a number, number within a range, email address, date, etc). | ||
This question type is an open-ended question. Use it if you want your respondents to comment on something in their own words. Questions of this type are good for gathering additional information. | ||
Use this question type to make your respondents leave feedback on different statements or evaluate different products, features, or services. You can select a pre-defined set of scales, or you can customize a scale of your own. | ||
Questions of this type allow you find out which item is most preferred by your respondent even if they were originally unsure about the answer themselves. | ||
The 3D Matrix questions allow you to get feedback on several different attributes associated with a product or service at the same time. It allows you to gather information on these attributes by requiring the respondent to answer using numbers, text or by choosing from a drop down box. | ||
Use this question type if you want to control total sum of values entered by respondents. These questions are often used to ask for percentages that will sum to 100%, or to check for values, like in the example below, not to total more than 24 hours. The total sum will be displayed and calculated automatically. | ||
If you want your respondents to upload files for an answer, use the File Upload question type. You can allow your respondents to restrict the file types, size and allow respondents to use the Markup and the Crop features with the uploaded files. Respondents can upload the following file types:

Object Lookup is a special question type that allows you to use Data Models or Contact Managers as a database and provide respondents with an option to select one record from them as an answer to your question. Use this question type if you want your respondent to choose a certain object from a dropdown by specifying its parameters in another dropdown if needed. Data in this dropdowns is transferred from selected columns of Data Models or Contact Managers created in your account. | ||

List of all available answer choices is displayed as checkboxes.
